If you require a patient to be discussed at the monthly Oncoplastic MDT please complete the following form and place it in the Red Oncoplastic MDT folder in JH's office.

This form will be used to document the discussion and outcome and when completed will be scanned to the merged folder and a hard copy will remain in the red folder.
